Before student-athletes walk onto the field, the state of Wisconsin requires them to walk into get a sports physical. But don’t worry; Klasinski is here to help out.
Klasinski is offering two complimentary sports physicals on July 18 and 30, well before athletes start practice for their favorite organized sports. All you’ll need to do is complete the required WIAA forms, which can be found here. You must also fill out a Privacy Practice Form and a Concussion Testing Letter. To get into the exam as quickly as possible, please bring the required forms filled out and signed prior to your appointment.
Appointments are encouraged and walk-ins are welcome on a first come, first serve basis. Appointments are available between 3:00-6:30 p.m. and walk-ins between 3:00-6:00 p.m. Call the clinic at (715) 344-0701 to make an appointment.
Athletes are reminded to wear eyeglasses/contacts, athletic shoes and have their WIAA preparticipation history and permission forms filled out.
A sports physical is an essential step to ensuring an athlete is healthy and safe during competition. This exam reviews your child’s physical capabilities to be able to participate in sports. A complete annual wellness exam, however, reviews all aspects of your child’s health and development.
Most insurance providers will cover the full cost of an annual wellness exam. If your child has not had a complete physical within the last year, the sports exam will be converted into an annual wellness exam. If you have questions about coverage of the physical exam, call the number on the back of your health insurance card.
